Form Builder Demo

The survey questions below demonstrate the questions and layout options you can create while using Form Builder.  Each question represents a different question type that can be used.  For instructions on how to use Form Builder, see https://kb.wisc.edu/wiscwebcms/page.php?id=37185 


This is an example of the Checkbox form field. It gives a single option for users to select. A sample questions might be: Are you a UW-Madison student?  

This is a Layout option called Paragraph Text. It allows you to add text without any option for an answers. It's a useful option when you have information that the user would need to know in order to complete the survey, but there is no information to ask. This Paragraph Text is surrounded by the Layout option called Separator. It allows you to add horizontal lines to your form to create natural breaks, making it easier for your users to read and comprehend.


This is an example of the Checkbox List form field. It allows you to let users select multiple answers. This question also uses the Layout option: Display in one column. A sample question might be: Have you taken classes during the following semesters?
  
  
  

This is an example of the Drop-down List. It allows you to have multiple answers in a drop-down menu and users can select one. An example question might be: What year in school are you?

This is an example of the Radio Button List. It allows you to have multiple answers but users can only select one. An example question might be: What year in school are you?  
  
  
  
  

This is an example of the Text Box. It allows you to have open-ended questions for users to answer. This question uses the Layout option: Display in 1 column. An example question might be: Please explain the issue you are having WiscWeb CMS in detail. Include any relevant error messages, browser version, and OS version.
